Dobutamine, dopamine and dopexamine are inotropes used in cardiac failure, shock and cardiac surgery. Ephedrine, metaraminol, noradrenaline and phenylephrine are used for acute hypotension. Adrenaline is used in cardiac arrest.
In all these acute clinical situations it is unlikely the mother will be breastfeeding, although the appropriate precautions will have to be followed if breastfeeding is resumed soon after drug administration.
Alternatives will primarily depend on the acute clinical situation, rather than the risk in breastfeeding.
